In this area are additional administrative policies that are important to know when visiting our
office. Payment Policy Insurance co-pays are due when services are rendered. We expect patients without insurance to pay at the time of service unless they make arrangements
prior
to their appointment. We accept debit card, credit cards, checks or cash. Paying
for services at the time they are rendered helps our office by reducing the amount of billing we need to do thus saving money
so that we can keep our costs down. Patient
bills that remain unpaid for 90 days may be turned over to collections and may result in discharge from the practice. Phone Messages When you leave a phone message, whether with a person or on voice mail, it may take several hours before
someone can return your call. Please be assured that you are important to us and we will do our utmost to return all calls
promptly. If you do not receive a call back within 24 hours, please phone again. The message may have been misplaced or we
have not been able to reach you by return call. Prescription
Refills Please
address all prescription refill requests at your scheduled appointment. If you find you need a prescription refill prior to
your next appointment, please have your pharmacy fax us a request and we will address these requests in a timely manner. Please
allow 24 hours for prescription refills to be completed. (For your safety and health, we do not prescribe
antibiotics or steroids over the phone.)Mr. Deweese will prescribe the medications that he determines to be the best fit for your condition
and allowed by your insurance. He will also be able to make referrals to specialists as needed.Supervising
PhysicianMr. Deweese
is supervised by Dr. Tracie S. Garmany, who is a family physician at Dollywood in Pigeon Forge, TN where she practices medicine
alongside her husband, David, who is a nurse practitioner. She received her education at East Tennessee
State University. A message can be left for Dr. Garmany at 865-868-1333.AppointmentsFor new patient appointments, please arrive
15 minutes prior to your scheduled time to fill out the necessary forms to help us to process insurance claims properly.
If you have received this paperwork in the mail, please have it completed before you arrive and present it to the front
desk upon your arrival along with your insurance card and any applicable co-payment.If you are unable to keep a scheduled appointment,
please contact our office if you are aware of the conflict. This will enable us to reschedule the time
for another patient.If three scheduled appointments are missed without contacting our office, this may warrant
dismissal from Dayspring. Technology In light of today’s ever progressing technology, our office
utilizes electronic medical records. This makes it simpler to send records for referrals and to keep track
of all patient contact with our office.
